As a Regional Center–vendored SLS provider, UHR is committed to promoting a person centered approach, independence, dignity, safety, and personal choice, while tailoring supports to meet each person’s developmental stage, goals, and unique needs.

Below is a detailed overview of what UHR SLS provides:

Daily Living & Independent Living Skills Development

UHR supports adults (and children) in learning and maintaining essential daily living skills, including:

Age-appropriate meal planning, food prep, and kitchen safety
Grocery shopping and household supply management
Household cleaning, organization, and home routines
Laundry and clothing care
Establishing healthy and safe home environments
Routines that build independence and confidence

Personal Care & Health Support

UHR provides non-medical support that varies by age and need:

Hygiene and grooming support
Bathing and dressing assistance when appropriate
Scheduling and coordinating medical, dental, and therapy appointments
Transportation to medical appointments when included in the IPP
Monitoring health concerns and assisting with follow-up
Medication assistance – Assistance with setting up medication organizers, tracking schedules, and coordinating prescription refills with pharmacies and providers, Medication reminders and support for medication self-management.

Community Access, Social Development & Integration

UHR promotes community participation for both youth and adults:

Age-appropriate community outings and recreational activities
Transportation training (public transit, paratransit)
Support building friendships and natural supports
Laundry and clothing care
Participation in school, day programs, employment programs, or extracurriculars
Culturally relevant community involvement

Financial Skills (Age-Appropriate)

UHR assists individuals in understanding and managing personal finances:

Teaching basic financial concepts to youth
Budgeting, money handling, and banking skills for adults
Bill management systems and reminders
Understanding benefits and financial responsibilities
Coordination with payee or financial support services as needed

Safety Training & Emergency Preparedness

UHR prioritizes safety for children and adults:

Individual safety planning
Teaching safety skills at home and in the community
Emergency preparedness (fire, earthquake, medical events, etc.)
Crisis support and urgent response
Assistance with obtaining emergency supplies

Housing Support & Home Stability

UHR assists individuals and families with housing needs:

Helping adults locate and secure independent housing
Supporting youth transitioning to adult living arrangements
Assistance with leases, applications, and move-in documents
Ensuring the home environment is safe, clean, and stable
Coordination with landlords and property managers
Support during home transitions

Communication & Self-Advocacy Development

UHR encourages individuals to express their needs, preferences, and goals:

Individual safety planning
Teaching safety skills at home and in the community
Emergency preparedness (fire, earthquake, medical events, etc.)
Crisis support and urgent response
Assistance with obtaining emergency supplies

Behavioral Support (Non-Clinical)

Within SLS scope, UHR provides supportive behavioral strategies:

Implementing strategies from Behavior Support Plans (BSP)
Reinforcing positive routines and coping strategies
Emotional regulation support
Collaboration with clinical behaviorists for specialized needs

Coordination of Care & Services

UHR communicates and collaborates with all members of the individual’s support network:

Regional Center service coordinators
Schools (for children), day programs, employment programs
Therapists, medical providers, and mental health providers
Family members, guardians, and natural supports
Participation in IPP and team meetings
Complete documentation and progress reporting

24/7 On-Call Support

UHR ensures individuals and families can access support after hours:

24/7 on-call emergency response
Guidance during crises or urgent situations
Additional safety support beyond scheduled visits

What UHR SLS Does Not Provide

To maintain clarity, UHR SLS does not provide:

Shift-staffed 24-hour care unless individually authorized
Medical or clinical procedures
Medication administration (unless separately licensed)
Payee services or conservatorship duties
Stand-alone transportation outside SLS goals
Clinical mental health or behavioral therapy
